How to Request assistance from OHIA
OHIA products and services can be requested through a number of ways. Depending on your request, you may need to submit one of the following:
- Access Requests
- Service Requests
- Incidents / Issues
- New Project Requests (NPRs)
Access Requests

The following document contains procedures that all users must follow in order to obtain access to OHIA-supported databases, as well as authorization to export, print, or manipulate in any manner, data from the databases. Users must diligently safeguard sensitive data and Restricted Information in accordance with UCLA Health information security and privacy policies. Violations of such policies will result in immediate removal of access privileges and could result in disciplinary action.
Service Requests
Service requests can be used to request many of OHIA services. Typical requests that can be completed through a service request are:
- New and/or changes to existing Clarity/Crystal Reports
- Scheduling of a Report
- Requests under 100 hours of effort
Incidents/Issues
Incidents are used to report when an OHIA Solution is broken or not working as expected. Requests to report an issue can be submitted via a ServiceNow Ticket
New Project Requests (NPRs)
A new project may need to be submitted for the following types of requests:
- Web-based Dashboards
- Self-Service Analytics
- Requests over 150 hours of effort

Once the form has been completed, please email to OHIAPMO@mednet.edu. Your NPR will be reviewed by the appropriate Prioritization Committee and a Project Manager will reach back out to you with the decision.
