Federated computing promises to unlock collaborative medical research while preserving data sovereignty. The reality is messier. 

This talk offers a candid practitioner’s view of implementing federated AI systems for rare disease research—from the diagnostic pipeline to molecule design—including the coordination overhead, the technical debt, and the political minefields that rarely appear in architecture diagrams. 

For institutions beginning their federated journey, I’ll share lessons from the rare disease community, where researchers have been pushing this envelope out of necessity rather than choice.
