What is Qualtrics?

Qualtrics is a HIPAA and FERPA-compliant online survey platform that allows you to easily create a survey, collect and store data securely, analyze responses, and present results using professional-quality graphs. It is available to UCLA Health faculty, staff, and students at no cost. The service can be used to support academic research, teaching, institutional business, and to reinforce key decisions with insights from colleagues, customers, employees, and more.

Access Qualtrics

How do I access Qualtrics?

The tool is free to our community, with no setup required.

  • Visit the UCLA Health Qualtrics page
  • Log in using your Mednet AD credentials
  • On your first visit, select "I don't have a Qualtrics account"
  • Accept the Terms of Service and start creating surveys

Quatrics Training

Qualtrics provides a lot of support material, on-demand training, and live training sessions through their XM Basecamp. The following will provide instructions on how to access Qualtrics XM Basecamp.

To access Qualtrics training:

  1. Go to

  3. Type in "uclahs" as your Organization ID.
  4. Log in with your AD credentials.
